TO THE II1LT--ACASE IN SURGERY1
San Fianclte Argonaut.
Looking nt my frlonil ns ho Iny upon
my bed , with tlio jowolcd knife hnmllo
protruding from hia breast , I bcllovcd
that lie was clyinpt.
Would the physician novel * come ?
"Pull it out , old follow , " bopped the
BUfforor , throuph white , drawn lips , his
paflpinp voice being hardly less distress
ing than the unearthly loolt In his oyos.
"No , Arnold , " said I , as I hold his
hand and pontly stroked his forehead.
It may have boon instinct , a certain
knowledge of anatomy , that made mo
refuse.
"Why not ? It hurts , " ho gasped. It
was pitiful to st'o him Butter , this strong ,
healthy follow.
The resident physician walked in a
tall , grave man , with gray hair. Ho
wont to the bed , and 1 pointed to the
knife handle , with its irroat bold ruby in
the end and its diamonds and emeralds
alternating In quaint designs in the
bides. The physician started. lie felt
Arnold's pulse and looked nu//led.
"When was this donoV ho asked.
"About twenty minutes ago. " I an
swered. . . .
The physician started out , beckoning
mo to follow. , ,
"Slo D ! " said Arnold. Wo obeyed
"Do you wish tospoak to moV' he asked.
"Yes , " replied the physician , hesitat
ing.
ing."Sneak in my presence , then ; I fear
" " said the , "if
"Then , physician you
have any any matters to to adjust ,
they should bo attended to at once. I
can'tlo nothing for you. "
"How long can I livo.J asked Arnold.
The physician thoughtfully stroked
his gray beard. "It depends , " ho ilimlly
said , "if the knife bo withdrawn you
may live three minutes ; if [ I bo allowed
to remain you may possibly live an
hofcr notlo'ngfr. "
Arnold never Ilinched. Tt was not the
first time lie had faced death , which had
"Thank you"ho said smiling
through his pain ; "my friends will pay
you. I Inivo Homo things to do : Lot
the knife rotniiln. " Ho ttirnotl his eyes
to inino , anil , pressing my liand , said
affectionately , "and I thank you , too ,
old follow , for not pulling it out. "
The physician , moved by a SOIIPO of
delicacy , loft the room , Buying : "King
If there in u change. "
Ho had not gene far when ho turned
nnd came back.
" " said ho "but there is
"Pardon mo , , n
young surgeon in the hotel who is said
to bo a very skillful man. My specialty
is not surgbrv. May I call him'r'
"You , " eaid I , eagerly ; but Arnold
smiled and shook his head. "I fear
there will not bo time , " ho said. But I
refused to heed him , and directed that
the surgeon bo called immediately. I
was writing at Arnold's dictation when
the two men on to red the room.
There was something of nerve and
assurance In the young surgeon that
struck my attention. His , manner ,
though quiet , was bold and straightfor
ward , and his movements sure and quick.
Thcso are general peculiarities of Highly
educated young surgeons. This young
man had already distinguished himself
in tlio performance ol some dllllcult hos
pital laparotoinles , and ho was at that
sanguine ago when ambition looks
through the spectacles of experiment.
And tlion , xoal and ambition are often
identical. Dr. Htioul Entrefort was the
newcomer's name. Ho was a Creole ,
email and dark , and ho had traveled
I ' nnd studied in Europe.
"Spoalc freely , " gasped Arnold , after
Dr. Entrofort bad made an examination.
"What think you , Doctor ? " asked En
trofort of the older man.
"I think , " was the reply , "that the
knifoblado has penetrated the ascending
aorta about two Inches above tbo heart.
As long as the blade remains in the
wound the escape of blood is compara
tively small , though certain ; were the
blade withdrawn , the heart would almost
instantly empty itself through the
nortal wound. "
Meanwhile , Entrofort was deftly cut
ting away the undershirt , and soon had
the breast exposed. Ho examined the
coin-studded hilt with the keenest in
terest.
"You tire proceeding on the assump
tion , doctor"ho said , "that this weapon
is a knifo. "
"Certainly , " answered Dr. Rowoll ,
Giniling ; "what else can it boV"
"It is n knife , " faintly interposed
Arnold.
"Did you seoitV" Entrofort asked him ,
mtieklv.
" 1 did for a moment. "
Entrefort shot n quick look at , Dr.
Rowoll , and whispered : "Then it is
not suicide. " Dr. Howell nodded.
"I must disagree with you , gentle
men , " qulotly remarked Entrofort ;
"this is not n knifo. " Ho examined the
handle very narrowly. Not only was
the blade entirely concealed from view
within Arnold's body , but the blow hud
boon so strongly delivered that the skin
was depressed by the guard. "Tho fact
that it is not u knife protfunts n very
curious series of facts and contingen
cies , " pursued Entrefort , with amazing
coolnosj , "some of which are , so far as I
am Informed , entirely novel in the his
tory of surgery. "
A quiz/teal expression , faintly amused
nnd manifestly interested , was upon Dr.
HowoU'tf face.
"What is the weapon , doctor ? " lie
nskcd ,
"A stiletto. "
Arnold started. Dr. Ilowoll appeared
confused , "I must confess , " ho said ,
"my Ignorance of the diU'oronces among
these penetrating weapons , whether
dirks , daggers , stilettos , ponlrads , or
bowie-knives. "
"With the exception of the stiletto , "
explained Entrefort , "all the weapons
you mention have onu or two edges , so
that in penetrating they cut their way.
A stiletto IH round , is ordinarily about
half nn inch or loss in diameter at the
guard , and tapers to u sharp point. It
penetrates solely by pushing the tissues
nsldo. You will understand the import-
mice of that point , "
Dr. Uowoll nodOed , more deeply interested -
torosted than ever.
"How do you know it is a stiletto ? " I
nskcd.
"Tho cutting of these stones is the
work of Italian lapidaries , " ho said ,
"and they wore sot In Genoa. Notice ,
too , the guard. It Is much broader and
shorter than the guard of an edged
weapon , in fnet , it is nearly round. This
weapon is about 100 years old , and
would bo cheap at 120,000 llorins. Oh-
eerve. nlbo , the darkening color of
vour friend'B breast in the immediate
vicinity of the guard , this ituiicntos that
the tissues have been bruised by the
crowding of the 'blade , ' if I may use the
term. "
i >
"What has allthls to do with wo ? "
gasped Arnold.
"A simple ray of hope to your despor-
ntu condition. "
Arnold's eyes s parktcd nnd caught
his breath. A tremor passed all through
him , and I felt it In the hand 1 was hold
ing. Life was sweet to him after all
this dare devil who had just faced death
with such calmness.
"With your permission , " said Entro
fort , addrorislng Arnold. "I will do what
1 can to save your life. "
"You may , " said the poor boy.
"And ovou If I succeed ( the chance s
one In a thousand ) you will never bo a
BOUi'tl man , ami a constant nnd terrible
danger will always bo present. " Entro-
fort wrote n short note and sent it away
by a hastily summoned boll boy.
"Meanwhile " resumed " life
, ho. , "your
is in Imminent danger from shock , nnd
the end may nome In a few minutes or
hours from'that cause. Attend without
delay to whatever matters may require
settling , nnd Dr. Howell , " glancing at
'
that go'ntlcman , "will give you some
thing to brace you up. I speak frankly
for I see you are a man of extraordinary
norvo. "
Dr. Rowcll , evidently bewildered by
his cyclonic young associate , wrote a
prescription , which I sent by a hey lobe
lllled. With unwise zeal I asked Entro-
"Is there not danger of lockjaw ? "
"No , " ho replied.
I subsided. The man's coolness was
nmn/.tng.
Dr. llowcll's medicine cnmo , nnd I ad
ministered a dose. The physician and
the surgeon then retired. The poor
sufferer strnightod up his business.
When it was done ho aMccd mo :
"What is that crazy Frenchman going
o do to me ? "
" 1 liavn no idea ; bo patient. " '
In loss than nn hour they returned ,
bringing with them a keen-eyed , tall
voting man , who had a nurViboi * of tools
wrapped in an npron. Evidently ho was
unused to such scenes , for ho became
deathly pale upon seeing the ghastly
spectacle on my bed. Wis staring eyes
ho began to retreat toward the dooY ,
stammering :
"I I can't do it. "
"Nonsonso , Hippolyto ! Don't bo a
baby. Why , man , it is a case of lifo and
' 'Hut look at his oycsl Ho is dying ! "
"No more nonsense , my boy , it must
be done. "
"I have to make some preparations
before you may begin. Hippolyto , and I
want you to observe mo , that you mny
become used , not only to the sight ol
fresh blood , but also , what Is more try
ing , tlio odor of it. "
llippolvlc shivered. Entrefort opened
a case of surgical insirumonis.
"Now , doctor , the chloroform , " ho
said to Dr. Uowoll. .
"I will not take it , " promptly inter
posed the sufferer ; "I want to know
when I die. "
"Very well"said Entrefort ; "butyou
have little nerve now to spare. Wo
will try it without clilorofo-m , how-
over. It will bo bettor if you can do
without. "
"What are you going to do.J" naked
Arnold.
"Save your life , if possible. "
"How ? Toll mo all about it. "
"Vorv well. then. The point of the
stilotto'hns passed entirely through the
aorta , which is the great vessel rising
out of the heart , and carrying the
umitcd blood to the arteries. If I should
withdraw the weapon the blood would
rush from the two holes in the aortuanil
you would soon be dead. If the woanoti
hud boon a knife , the parted tissue
would have yielded and the blood would
have been forced out on either side of
the blade and would have caused , death.
As it is , not a drop of blood has escaped
from the aorta into the thoracic cavity.
All that is loft for us to do , then , is to
allow the stiletto to remain permanently
in tlio norta. It is a desperate chance ,
but it is the only chance. The fact that
Iho weapon is a stiletto is the important
fact a stupid weapon , but a blesssing to
us now. If the assassin had known more
she "
Upon his employment of the noun
"assassin , " and the feminine pronoun
'she , " both Arnold and I' started
violently , and I cried out to the man to
stop.
"Lot him proceed , " said Arnold , who ,
by u remarkable effort had calmed him
self.
self."Not
"Not if the is "
subject painful , Entro
fort said.
"It Is " Arnold. "
not , protested . "Why
do you think the blow was struck by a
woman ? "
"Beciiuso , ilrst , no man capable of
being an assassin would carry so gaudy
and valuable weapon ; second , no man
would bo stupid enough lo carry so
antiquated and inadequate a thing as a
stilotto. She was a strorfg woman , too ,
for it requires a good hand to drive a
stiletto to the guard , even though it
miss the sternum by a hair's breadth
and slip between the ribs. She was not
only.a strong woman , but a desperate
ono also.
"That will do , " said Arnold.
"Then , " resumed Entrefort , "I shall
tell you what I intend to do. First ,
however , ! must congratulate you on the
fact that , as the weapon may not bo
withdrawn , it did not enter the heart
instead of the aorta , for if left in the
heart inflammation of the tissues would
follow and produce death. Tlio danger
exists oven as it is. There will un
doubtedly bo in Humiliation of the aorta ,
which , if it persists , will cause a fatal
aneurism bv the breaking down of the
aortal walls , but wo hope , with the help
of your youth and health to chock it.
"Another serious dilllculty is this :
With every inhalation the enure thorax ,
or bony structure of the chest , consid
erably expands. The aorta remains
stationary. You will 300 , therefore ,
that as your aorta and your breast are
now hold in relation to each ether by
the rigid stiletto , the cliost , with every
inhalation pulls the aorta forward out
of its place about half an inch. I um
certain that it Is doing this , because
there is no indication of an escape ot
arterial blood into the thonicic cavity ;
in ether words the mouths of the two
aortal wounds have seized upon the blade
with a linn hold , and thus prevent it
from slipping in nnd out. This is a very
fortunate occurrence , but ono which will
cause pain for some time. The aorta ,
you may understand , pulls the heart
backward and forward with every breath
you take , but that o"gun , though now
much surprised , will accustom itself to
its now condition.
"What 1 fear , however , is the forma
tion of a clo around the blade. You see ,
the presence ) of the blade in the aorta
has already reduced the Wood carrying
capacity of that vessel ; n clot , therefore ,
need not be very largo to stop up the
aorta , and , of course , if that should
occur death would ensue. You are be
ginning to reullzo some of the dangers
which await you , "
Arnold smiled faintly.
"Hut wo shall do our best to prevent
the formation of a clot , " continued En
trofort ; "thoro are drugs which may bo
used with effect. "
"Aro there more dangers ? "
"Many more , some of the more ser
ious have not been mentioned. One of
those is the liability of the aortal tissues
pressing upon the weapon to relax their
hold and allow the blade to slip. That
would let out the blood and cause
death. "
"Is that all ? " asked Arnold.
"No ; but Is not that enough ? "
"Moro than enough , " said Arnold ,
with a sudden and dangerous sparkle in
his eye. Heforo any of us could think ,
the desperate fellow hud seized the
handle of the stiletto
with both hands in
a determined effort to withdraw it and
die. I had had no tlmo to order my
faculties to the movement of a muscle ,
when Entrofort with incrodlblo alert
ness and swiftness , had Amold'a wrist.
Slowly Arnold relaxed his hold.
' There , nowl" said Entrofort uooth-
Ingly , "that was a careless net and
might have broken the adhesion ? '
Then ho whispered hurriedly to
Arnold : "If you do that * ' with a mo
tion toward the hilt "I will have her
hnngod for murder. "
Arnold , almost choking , and with a
look of horror , withdrew his handstook
ono of mine in both of his , nnd placed
them on the pillow above his head.
"Proceed with your \Vork , * ' ho snld to
Entrofort.
Entrefort's hand was quick and euro ,
nnd ho used the knife with marvelous
dexterity. First ho made four r.quldls-
taut incisions outward from the guard
and just through the skin. Arnold
winced and hold lib breath at the Ilrst
cut , but scon regained command of him
self. Each incision was about two
Inches long. Quickly was the skin
peeled back to the limit of the incisions.
Tills was excruciatingly painful. Ar
nold groaned nnd his hands became
moist nnd cold. Down wont the knife
into the flesh , and blood flowed freely.
Dr. Uowoll sponged it off.
The swift knife went again at work.
Arnold's marvelous nerve was breaking
down. Ho clutched my hands with un
conscious strength. His eyes danced.
His mind was weakening. Almost in a
moment the ilesh had been cutaway and
removed down to the bones , which wore
exposed two ribs and the sternum.
"To work , Hippolyto bo quick ! "
He had evidently been coached. With
slender , long-lingered hands , which
trembled at first , ho selected certain
tools , made some quick measurements
of the weapon , and bi'gan to adjust n
queer little machine.
"What ' ' Arnold started so say ,
but n deeper pallor settled on his face ,
his hands relaxed , his voice was hushed ;
and his eyes closed.
"Thank God ! " exclaimed Entrefort ;
"ho has fainted. Ho can't stop us now.
Quick , Hippo'.ytc ! "
The mncainist attached the queer little
machine to the handle of the weapon ,
grasped the handle with his loft hand ,
and with his right began a series of
short , sharp , quick motions backward
and forward.
"Hurry , Ilippolytol" cried Entrofort.
"Tho metal is very hard. "
In a moment more something snapped1
Hippolyto started. Ho picked up the
handle of the stiletto. He had cut it off ,
leaving the blndo inside.
"Good , Ilippolylol" o xclaimcd Entro
fort. Almost in a ininuto the bright
end of the cut metal was closed from
view by the skin flaps , and these sowed
ogcther.
Arnold returned to consciousness and
glanced down nt his breast. Ho looked
puzzled.
"Whoro is the thing ? " ho linked.
"Hero is part of it , " explained Entre
fort.
fort."And
"And the blade - "
"Is an irremovable part of your inter
nal machinery. It had to bo cut off. "
resumed Entrofort , "not only because it
would bo troublesome and an undesira
ble ornament , but also because it was
necessary to remove tiny possibility of
withdrawing it. "
Arnold said nothing.
"Iloro is a proscription , " said Entre
fort ; "take the medicine as directed
without fail. "
"What for ? I see it contains muriatic
acid. "
"I may explain ten years from now. "
"If I live ? ' !
"If you live. "
Arnold pulled mo down to him and
whispered :
"Tell her to fly at onco. "
* * * * *
I thought I recognized a thin , pale
bright face among , the passengers wiio
wore leaving an Australian steamer
which had j'ust arrived at San Francisco.
"Dr. Entrofortl" I called.
"Ah ! " ho said , peering up into my
face ; "I know you now , but you have
changed. You remember I was called
'
nway'immediately after I performed that
cra/.y ooeration on your friend , and have
spent the intervening seven years in
India and China. But wasn't that the
most absurd , hare-brained experiment
that I tried on your friend ? I dropped
all that kind of nonsense long ago. Poor
follow , ho bore it so bravely ! Did ho
suffer much ? How long did ho live ? A
"Sovon years. "
The man staggered. "Incredible ! " ho
said.
said."It
"It is true ; you shall see him. "
"Toll mo about him , " ho asked ,
eagerly.
"Well , the change In him Is shocking.
Imagine a young dura-devil of twenty-
ono , who had no greater fear of danger
and death than of a cold , now a cring
ing , cowering man of twonty-oighs ,
nursing his lifo with pitiful tondornest ,
fearful that any moment something may
happen to break the hold of his aorta o
cliondriac , peevish , melancholy , un
happy in the extreme. The constan *
danger has worn out the last shred of
manhood ; left him a wreck. Can notht
ing bo done for him ? "
"Possibly. Lot us find him. Ah ,
there comes my wife to moot mo ! She
arrived on the ether steamer. "
I recognized her instantly.
"Charming woman"said Entrofort ,
and you'll like hor. Wo were married
four years ago at Bombay. "
Then ho introduced us. To my un
speakable relief she recognized neither
my name nor my faco. I must have ap
peared a peculiar- person to her , but it
was impossible to bo perfectly noncha
lant Wo wont to Arnold's rooms , I
with painful fear. 1 loft her in the
reception room and took Entrefort with
in. Arnold was too greatly absorbed
with his own troubles to bo dangerously
excited by mooting Entrofort , whom ho
greeted with indifferent courtesy.
"But I hoard a woman's voice , " ho
said , and before I could move ho had
i ono to the reception room , and ho stood
face to face with the beautiful adventur
ess who , wickedly dosnortito , hud driven
a stiletto into his vitnfs in a hotel seven
years before because ho refused to marry
hor. They recognized each other
Both started and turned pale , but sho-
qulekor wilted , recovered her compos
ure nnd extended her hand.
Ho staggered back , his face ghastly
with fear.
"Oh ! " ho cried out , "tho blndo has
slipped out I felt it full the blood Is
pouring outfit burns I um dying ! "
mil ho fell into my arms and instantly
ex nlred.
The autopsy revealed the astonishing
fact that there was no blade in him at
all. It had been gradually consumed by
the murlatlu acid which Entrofort hud
[ ) roscribod for that purpose , and with
ivhleh Arnold had kept his system con
stantly lllled , and the wounds In the
lorta hud closed In steadily with the
wasting blade and were perfectly healod.
All his vital organs were sound. My
| )0or friend , once so rcckloss and brave ,
md died simply of a groundless fear of
a woman , and she unwittingly had ac
complished her revenge.
