THIS CARD COULD HELP SAVE YOUR LIFE. NO ONE PLANS A MEDICAL EMERGENCY. Last year more than 45,000 medical emergencies occurred in the Lincoln area, and most with no advance warning. It's a frightening thought, but it can be a lot less frightening if you’re prepared and know where to turn. The health professionals at Lincoln General Hospital want you to be prepared for the medical emergency that could happen to you or your family. That’s why we’ve developed the Emergency Medical Information Card. Fill in both sides of this card and carry it with you at all times. The card provides identification and contains important information concerning allergies, medication and other data about your medical history. This information will help the Emergency physician provide the necessary medical care. To receive your free Emergency Medical Information Card, contact Lincoln General Hospital. Then you'll be more prepared if an emergency strikes. To obtain your FREE card call 473-5115 EMERGENCY/TRAUMA SERVICES LINCOLN GENERAL HOSPITAL 2300 South Kith Street • Lincoln, NE 08502
